Application Steps
To apply for enrollment at our Museum Campus in Lehi please follow these steps carefully:
1) Complete the New Student Application below
A non-refundable $90 application paid by credit card is required to submit your application. (Please note, this form may not display properly on some mobile devices, we recommend using a tablet or computer).
2) After you sign and submit the application you MUST open the email from RightSignature.com to verify your signature by clicking on the emailed link.
Your application is not submitted until you complete this step of clicking the link in the email. The email listed on the top of your application is the address that your verification email will be sent to. Please check your junk folder if you are not seeing the message.
Once you complete these two steps your application has been submitted successfully and time stamped.
We will work hard to review all applications as quickly as possible. Dancing Moose will communicate via email by to verify your child’s enrollment or waiting list status.
